    Virtual Integrated Care Team Training

    Southcentral Foundation’s (SCF) Nuka System of Care is based on relationships, trust, partnering, and the concept of shared responsibility. SCF understands that patients (known as customer-owners at SCF) are in control of their health. SCF has received awards for its innovative approach and is recognized worldwide as a model for health care delivery, operational efficiencies, and relationship-based services. In this two-day training, you will explore the systems and protocols SCF has developed to create effective partnerships and integrate services, and practice skills needed to improve team efficiency, satisfaction, and the customer experience through interactive, subject matter expert-led panels and sessions.

     WHAT YOU'LL LEARN 

    At the end of this training, you will be able to: 

    • Implement team-based best practices in the technical day-to-day delivery of care. 
    • Recognize your relational styles and the impact of different work energies on team dynamics. 
    • Utilize a personal toolbox developed in the training to support self-awareness, navigating resistance, and handling conflict. 
    • Facilitate communication to manage change and workflow in an integrated care team. 

    WHO SHOULD ATTEND

    SCF’s Integrated Care Team Training is useful for health care leaders, clinical managers, operations managers, improvement personnel, and anyone else looking to learn more about implementing integrated primary care teams at their organization.

    • $399 for groups of 5 or more

    CONTINUED EDUCATION

    In support of improving patient care, this activity has been planned and implemented by Alaska Native Medical Center (ANMC) and Southcentral Foundation. ANMC is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

    The ANMC Joint Accreditation CE Program Portfolio additionally supports Behavioral Health (APA), Social Work (ASWB-ACE), and Dietitians (CPEU).
