• Excellent Survey Performance: Demonstrate a high level of compliance with three or fewer findings on surveys, showcasing a dedication to exceptional care and service.
• Significant Improvement in Star Ratings: Achieve an improvement of one star or more in patient satisfaction or quality star ratings from 2023 to 2024, indicating a notable enhancement in care quality and patient experience.
